  Job responsibilities include, but are not limited to: Masters level social worker, working closely with the Clinical Team as a Care Manager providing ongoing mental health services to students, and supporting UHS staff through planning, implementing, and coordinating resources based on individual student and staff needs.

The goal of this position is to promote continuity of care through coordinating, collaborating and advocating based on students’ needs Coordinates care with students receiving services from University Health Services, CAPS and area hospitals Assists UHS and CAPS staff with the hospitalization process, discharge planning, follow-ups, and coordination of other services Establishes, maintains and coordinates relationships with UC services such as CAPS, Stetson Psychiatry, and other campus organizations that would assist with the students’ needs Consults with students to determine their needs and best course of treatment Provides transitional support to referring therapist and clients in need of referral to community services Maintains community referral resources Acts as liaison and coordinates between UHS and community referral sources, including hospitals, private practitioners, clinics, psychiatry, and other mental health agencies while maintaining a database of community referral resources.

Work cooperatively with CAPS and Stetson to develop a single access point for mental health services for all UC students.

Other duties as assigned   Minimum Qualifications: Master’s, Licensed Independent Social Worker Degree must be in social work Experience must include at least one year of post-master’s counseling and case management   Additional Qualifications: Familiarity with third party payer systems Familiarity with college student health Familiarity with demographics of UC, including international, LGBTQIA, and nontraditional students Demonstrated commitment to equity and inclusion   The University of Cincinnati, as a multi-national and culturally diverse university, is committed to providing an inclusive, equitable and diverse place of learning and employment.
